Весьма перспективным направлением оптимизации использования АМП может быть и сокращение длительности назначения препаратов. Так, например, рекомендуемая продолжительность назначения антибиотиков при НПивл составляет 14–21 день, тем не менее, показано, что адекватное лечение приводит к значительному улучшению по всем клиническим параметрам в среднем уже на 6-й день. Установлено, что в течение второй недели антибактериальной терапии обычно происходит колонизация дыхательных путей синегнойной палочкой и энтеробактериями, таким образом, длительная терапия является фактором риска колонизации резистентными возбудителями, что может провоцировать повторный эпизод НПивл [39-41].

Понимание актуальности проблемы заметного роста устойчивости микроорганизмов к антибиотикам на фоне относительной стагнации в создании новых препаратов привело к переоценке практики их использования. Исследование тенденций роста резистентности, изучение феномена «параллельного ущерба» и законов распространения «проблемных» микроорганизмов привели к формулированию различных по эффективности принципов рационального использования АМП, реализующихся в локальных и глобальных стратегиях их применения. Логично, что специальные мероприятия – программы по надзору за использованием АМП являются основным способом как сдерживания резистентности возбудителей и минимизации параллельного ущерба, так и средством существенной экономии денежных средств, затрачиваемых на здравоохранение [42].

Обычно подобные программы состоят из ряда рекомендаций, а последние, исходя из принципов доказательной медицины, имеют различные силу и качество доказательной базы [43]. При выборе стратегий, направленных на сдерживание роста устойчивости возбудителей к антибиотикам, следует остановить свой выбор на тактике проспективного аудита. Подобный подход основан на оценке практики использования АМП с интервенционной составляющей (например, с внедрением протоколов терапии или профилактики инфекций), а также на обратной связи и взаимодействии между клиницистами и специалистами комитета по надзору за использованием антибиотиков. Более того, в данной стратегии предусмотрена возможность широкого внедрения де-эскалационной терапии.
